Your situation

Do you already have an Arizona Travel ID (a license or ID with a star in a circle)?

A non-Travel Arizona license or ID says it is not valid for official federal purposes. An out-of-state license doesn't count.

Are you a U.S. citizen?
Is your current legal name different from the name on your identity document?

For example, your birth certificate shows your name before marriage.

Your plan

  1. Gather your documents

    • Accepted documents (bring 1)
      • U.S. birth certificate (issued by a U.S. state, territory or possession; hospital certificates and California Certified Abstracts of Birth are not accepted)
      • Consular Report of Birth Abroad (U.S. Certificate of Birth Abroad)
      • Unexpired U.S. passport or passport card
      • U.S. Certificate of Naturalization
      • U.S. Certificate of Citizenship
    • Accepted documents
      • Your complete Social Security number
    • Accepted documents (bring 2)
      • Utility bill
      • Credit card or bank statement
      • Insurance policy
      • Government document
      • Rental agreement
      • Any MVD document mailed to you
      • Arizona Residency Affidavit (form 40-5143) (counts as one of the two; you still need one other document)
  2. Start at AZ MVD Now

Sign in to AZ MVD Now to see if you can apply online or to begin your application, and update your address there first if it has changed. You can schedule an office appointment there; walk-ins are also welcome at MVD offices. If you have no Arizona license or ID, go straight to an office with your documents.

Common reasons people get turned away

  • A hospital birth record or a California Certified Abstract of Birth instead of a certified birth certificate.
  • Two proofs of residency from the same source, or without your current Arizona residential address.
  • Copies that aren't originals or certified by the issuing agency, or documents that aren't in English.
  • Applying too close to a trip: the card comes by mail and can take up to 2 weeks.
  • New to Arizona and getting a driver license: you take the tests unless you show an out-of-state license or driving record (MVR) expired less than one year.
