Common reasons people get turned away

  • Bringing only one proof of Ohio address, or two from the same source.
  • A metal Social Security card, or a W-2 or pay stub with a handwritten number.
  • Bills or statements older than 12 months.
  • A name change without the right document: the marriage record as an original or certified copy, a divorce or court order as a certified copy.
  • A license expired more than six months: you need a temporary permit and all required tests first.
